Implementing National Health Reform in California: Chronicling the Changes to Public and Private Coverage
The Affordable Care Act aims to transform the way health care is provided in the United States, and state officials are now grappling with the many challenges that arrived with its recent passage. Sweeping in scope, with an implementation timeline spanning nearly a decade, the new law ultimately will bring coverage to millions of previously uninsured citizens and create new coverage options for millions more.
To help understand the quickly changing landscape, the California HealthCare Foundation (CHCF), in partnership with Manatt Health Solutions, has assembled an initial assessment of the work ahead, Implementing National Health Reform in California: Changes to Public and Private Insurance.
The report focuses on the state's responsibilities in both the public and private coverage spheres, which will grow considerably. These include:
- Expanding Medi-Cal and reconfiguring eligibility standards under the program;
- Creating a health insurance exchange; and
- Implementing a wide range of mandated reforms to the commercial insurance markets in California.
Implementation of national health reform would be daunting in the best of times. Yet California, like many states, is operating with severe budget deficits that are expected to continue for years to come. Resources are already stretched thin in the very state agencies key to the effort. At the same time, a political transition at the highest levels of state government is in the near offing with the Governor's race already under way.
Despite these challenges and considerations, the new law has the potential to bring health insurance to many more Californians, thus improving their access to care. The goal for this early assessment is to help inform policymakers of actions needed to realize this potential.

Health Reform -- What Does It Really Mean?
Now that health reform has finally passed, we have a chance to reflect on what it really means. Is it a major victory for progressives? Or a huge subsidy to insurance companies?
The answer is clearly "yes" and "yes". What we've accomplished so far is to commit our society to provide health care to 32 million more people, at the cost of 1 trillion dollars over ten years. It also contains major initiatives in prevention, wellness, quality improvement, and long-term care. That's a huge step forward -- one of the most significant expansions to the social safety net in U.S. history -- and provides a platform to build on for the future.
Are the insurance companies getting a great deal? Yes, but at the cost of making their current business model illegal. They can no longer make money by cherry-picking the most profitable customers, and dumping the ones that really need help. And they are limited in the amount of administrative overhead (including profits and executive salaries) they can charge customers. Of course, effective regulation is needed to make this actually happen, and the ball will be in the court of the Obama Administration to do that. But with the new business model, insurance companies will have to compete on the basis of the value they provide to customers, rather than by being the best at getting rid of the sickest patients.
Can we stop working on health reform now that this legislation has passed? No. Most importantly, now that we're covering many more people, we really have to work on the problem of health care costs. The reform law contains a number of very solid cost control initiatives (you can find a great summary in this letter from 23 economists to the President), but they need to be translated into actual changes to the system. And then down the road, Congress needs to stick with what they said they would do, supporting cost-cutting so long as quality can be maintained or improved.
To sum up, passing health reform is like the invasion of Normandy. The war isn't over but we've overcome a really huge obstacle. And if we hadn't ... to mix up the metaphors, it would have been Waterloo for all of us.

President Obama's Health Reform Proposal
President Obama's plan to bridge the differences between the House and Senate health reform legislation was released today. The White House has created a micro-website for it here, and you can read a ten page summary here. A very good Q & A titled "What Reform Means To You" answers basic questions about the impact of reform for different groups: people that currently have insurance, small business owners, people on Medicare, etc. The Center for American Progress created a helpful table summarizing the major aspects of House, Senate, and Obama plans here.
